If your concern is about one of your children and you plan to consult about him or her, please also print out and complete a Developmental Questionnaire, which will save us some time during our first meeting. |

How soon would I be able to meet with you? At the present time, I meet with clients in my Saint-Lazare office on Mondays, Tuesdays and Wednesdays.
I meet with clients in my Downtown Montreal office on Thursdays and Fridays.
If you are available earlier in the day (i.e., between 10:00 am and 3:00 pm), I will be able to schedule an appointment with you sooner. (Evening availability is limited, and preference is given to couples and families who have to synchronize several schedules.) |

| Where is your Saint-Lazare office located? My Saint-Lazare office is located on the main street in downtown Saint-Lazare, near the corner of Bedard, in the Complexe Professionnel St-Lazare. The address is 1965 Chemin Sainte-Angelique, Suite 203.
From Montreal, take Autoroute 40 West towards Ottawa. Take the new Exit 26 and turn left at the end of the exit ramp onto Daoust, which will become Bedard. Go about 4 km until you come to Sainte-Angelique. Turn right. The Complexe Professionnel St-Lazare will be on your right, just across the street from a stone church. Here is a map showing the location: Parking is located behind the building. The waiting room is located on the second floor just outside my office.

| Where is your Downtown Montreal office located? My Downtown Montreal office is located at 1535 Sherbrooke St. West, at the corner of Guy (which becomes Côte-des-Neiges north of Sherbrooke), a three-minute walk north from metro Guy-Concordia (take the exit on Guy). Buses that pass directly in front of my office are the 24, 66, 165, 166, and 535. (Bus schedules for these routes are available at STM, Société de transport de Montréal). My office is conveniently located for students of McGill and Concordia Universities and easily accessible from Montreal General Hospital and Montreal Children's Hospital..

| Can I make an insurance claim for my therapy fees? Many workplace insurance plans or Employee Assistance Programs (EAPs) cover between 50% and 100% of consultation fees, often up to a maximum of $500 or $1,000 per year, with a registered psychologist—that is, a psychologist who is, like myself, a member of the OPQ (Ordre des psychologues du Québec). I am happy to provide you with receipts: either after each session, at the end of the month, or at the conclusion of therapy. Please check your benefits plan to determine how much of the therapy will be subsidized. |

| What if I forget or have to cancel a session at the last minute? Therapy is an important undertaking which involves mutual commitment. I promise to reserve your scheduled time slot and to be present and available for you during the hour we have agreed upon. You promise to attend the session and to pay promptly for services rendered. In case of illness or a medical or family emergency, your absence is certainly understandable, and there will be no fee. If you need to cancel for any other reason, I will not charge for the session if you inform me of the cancellation by telephone 24 hours in advance. If you cancel within this time or if you do not come to the session, for whatever reason, I will charge you one half of the regular session fee. This is the standard practice recommended by the Ordre des psychologues du Québec. |
